James is working towards completing their probation by actively seeking out the counselling and remaining engaged with all their professional supports. James has expressed interest in enrolling into LEEP once these conditions are fully completed. Achieving full time employment is at the forefront of their goals and they are currently handing out resumes while waiting for further employment training. James is currently 1 year and 4 months sober and celebrates these huge successes with the natural supports and professional supports in their life.

Jay is working towards and achieving success every day. Jay reached out for support with their caseworker and identified that they needed further assistance in finding the right support for them. Jay is demonstrating initiative that they previously struggled with.  Jay has continuously talked about how full-time employment is their goal and consistently hands out resumes with their caseworker. Jay is on track to attending the next LEEP session available.

John recently expressed the need to attend detox as he has been using more than usual. Caseworker offered support but John said he could handle calling Alpha House Detox on his own. Caseworker later received a message from ‘s CUPS Nurse who stated that she received information that John acquired a spot at Alpha House Detox and showed up to check in at 3:00am. Caseworker was informed that John made the phone call, took transit, and checked in all on his own.

After months and months of attempts, caseworker and Jane were finally able to get her government ID. Jane is extremely street entrenched and struggles with substance use and has been very difficult to get a hold of. Caseworker wants to note that this was a major accomplishment for Jane because this is the first piece of government ID Jane has ever had besides her birth certificate.

Joe had been a program participant since November 2015. Over the past two years, the Caseworkers, Occupational Therapist and Housing Liaison had been supporting with developing a sustainable graduation plan for Joe. This included supporting with connecting to AISH benefits, life skills building around maintaining housing standards, part-time employment opportunities and managing housing related expenses. Joe moved into his own unit in NE from a Master Lease unit. The rent subsidy was gradually reduced to ensure he was able to maintain financial stability. He has connected with family for Natural Supports.
